Severe exacerbation of chronic heart failure following community-acquired pneumonia: A clinical case report

Abstract:
The aim of the study was described difficult clinical case in the 64-year-old male patient with a complex cardiovascular history who developed severe chronic heart failure exacerbation following community-acquired pneumonia, requiring intensive care management. We observed and treated the patient with a history of ischemic heart disease, essential hypertension, persistent atrial fibrillation, type 2 diabetes mellitus, and stage C chronic heart failure. Initial clinical evaluation and imaging confirmed bilateral polysegmental pneumonia complicated by grade 1 respiratory failure. Despite antibiotic and diuretic therapy, the patient exhibited progressive respiratory insufficiency, hemodynamic instability, and worsening renal function. Transfer to the intensive care unit was necessitated by refractory hypotension and severe heart failure decompensation, evidenced by an NT-proBNP level of 30,822 mg/mL. Advanced supportive therapy, including vasopressor support, intensified diuresis, corticosteroids, and antiarrhythmic treatment, led to gradual clinical stabilization. However, follow-up echocardiography revealed a decline in left ventricular ejection fraction (LVEF) from 53% to 38%, reflecting significant myocardial dysfunction. After 28 days of hospitalization, the patient was discharged with optimized pharmacological therapy and a recommendation for close outpatient follow-up. Results: this clinical case highlights the complex pathophysiological interactions between community-acquired pneumonia and chronic heart failure, emphasizing the critical role of early diagnosis, intensive monitoring, and a multidisciplinary therapeutic approach in optimizing outcomes. Given the persistent cardiac dysfunction observed in this patient, it is essential to consider community-acquired pneumonia as a potential precipitant of long-term myocardial impairment in high-risk populations. Further research is warranted to refine management strategies for patients with heart failure exacerbations secondary to respiratory infections.
