Clinical profile and outcomes of orbital mucormycosis in patients with type ii diabetes mellitus

Abstract:
Background: Orbital mucormycosis is a rare but severe fungal infection that predominantly affects immunocompromised individuals, especially those with Type II Diabetes Mellitus. This study aimed to investigate the clinical features, diagnostic challenges, and outcomes of orbital mucormycosis in this patient population.
Materials and Methods: A prospective, non-comparative cohort study was conducted over 12 months at the Department of Medicine & Opthalmology SVS Medical College, Mahabubnagar. A total of 20 adult patients with Type II Diabetes Mellitus and histopathologically confirmed orbital mucormycosis were included. A comprehensive clinical, ophthalmic, and radiological evaluation was performed, followed by a standardized treatment protocol involving antifungal therapy and surgical debridement.
Results: The majority of patients (60%) were in the 51-55 years age group, with a significant male predominance (80%). Painful proptosis was the most common clinical presentation (75%), followed by seropurulent discharge with black eschar (50%). Radiological findings revealed mucosal thickening in 80% of cases. Most patients (85%) had isolated orbital mucormycosis, with minimal systemic involvement. The study highlighted the need for prompt diagnosis and aggressive management.
Conclusion: Orbital mucormycosis in patients with Type II Diabetes Mellitus primarily affects older males and presents with significant ocular symptoms, notably painful proptosis. Early detection and treatment are essential to improve visual and survival outcomes. This study emphasizes the importance of managing diabetic patients with suspected fungal infections at the earliest possible stage.
